Hepatocellular Carcinoma (HCC)

Epidemiology

  • Most common type of primary liver cancer, mainly in males.
  • Risk factors include chronic liver disease (cirrhosis) and hepatitis B/C.

Clinical Presentation

  1. Asymptomatic
  2. Local symptoms: abdominal pain, jaundice, weight loss.
  3. General symptoms: fever, malaise, ascites.

Diagnosis

  • Imaging: Triphasic CT scans as the gold standard.
  • AFP levels may indicate HCC but are not definitive.

Management Approaches

Treatment Modalities for HCC

Curative

  • Surgical resection, liver transplantation (Milan Criteria).

Palliative

  • Local: RFA, microwave ablation.
  • Regional: TACE (Trans-arterial chemoembolization).
  • Systemic: Sorafenib.

Prognosis

  • Poor without treatment; approximately 5-year survival post-treatment is 18%.
